This link to the American Cancer Society website provides access to the following survivorship care plan tools: 1.) " What's Next: Life After Cancer Treatment," from the Minnesota Cancer Alliance.   2.) "Journey Forward," a collaboration from a number of leading oncology resources.  3.) "Prescription for Living," created by the Oncology Nursing Society, the American Cancer Society, and the University of Pennsylvania, among others.  4.) " ASCO Cancer Treatment Summaries," provided by the American Society of Clinical Oncology, the world's largest association of cancer physicians.  and 5.)  The Lance Armstrong Foundation " Livestrong SurvivorCare Program."  This post-treatment program provides medical and psychosocial tools for Survivorship recovery.
